High Peaks Hospice is seeking a Nurse Practitioner to deliver and support clinical care for our patients.

Position Description:
The Hospice Nurse Practitioner works in collaboration with the hospice team to provide hospice care services to patients and families in a variety of patient care settings. The Hospice Nurse Practitioner provides consultation, assessments, and evaluation of the plan of care. The Nurse Practitioner can also perform procedures for which he or she is privileged to do and document all assessments, consultations, and care. Home visits/evaluations, and inpatient evaluations are part of the Nurse Practitioner’s role.
Responsibilities:
- Collaborates with the Medical Director, Hospice Physicians, Executive Director, Hospice Care Coordinators (HCCs), and RN Case Managers to ensure quality and continuity of care across the continuum of services and locations of care.
- Attends Interdisciplinary Team (IDT) Meetings as requested.
- Visits assigned patients in their place of residence to evaluate their continued eligibility for hospice services under recertification protocol.
- On IDT request, visit patients to support the IDT process of assessment, care planning, and intervention.
- Documents all patient information and services under HPH policy and procedure.
- Adheres to standards of practice and all approved policies.
Qualifications:
- Master’s Degree or graduate certificate from an approved advanced practice nursing program.
- Current licensure as a nurse practitioner in the state of New York State.
- Demonstrated clinical competence in relevant clinical areas, particularly hospice, oncology, and family practice; hospice and/ or palliative care experience preferred.
- Ability to quickly learn new software (electronic medical record system).
- Valid NYS driver’s license and available automobile for travel within the hospice service area.
